print, sign & mail them to us.

To fill out and print online forms, you will need the free Adobe Acrobat Reader to open our PDF files. If you do not have the free Adobe Reader software installed on your computer, you can click on this logo to get it:

STEP 1:  Fill out and print the Registration form: CLICK HERE. There is a drop down menu on the form with which you can select one of the above referenced class dates.
STEP 2: Fill out and print the Orientation form: CLICK HERE.
STEP 3: Mail all forms to the HappyMe.com address, above.

When you arrive for class, you will also be required to complete class-related paperwork prior to class time.
